USER ENROLLEE SERVICE AGREEMENT
Please read the Terms and conditions carefully before continuing with the registration. These general Terms and Conditions apply to all registered enrollees under IMSHIA. By registering under IMSHIA, you agree to be bound by these Terms and Conditions. If you do not accept all of these Terms and Conditions, do not register.
The Imo State Health Insurance Scheme is available for all residents of Imo State, who are both in the Formal and Informal Sector.
By accepting, you are bound by the entirety of these Terms and Conditions.
Enrollees in the Formal sector are to pay a premium of 5% of their basic salary which will be deducted from source to cover both the spouse and four dependants of his.
Enrollees in the Informal sector must pay an annual fee of N15,000 per individual.
Your insurance plan will automatically terminate at the expiration of one year from the date of registration (For the Informal sector).
At the attainment of 60 years of age or 35 years in service, whichever comes first, your health insurance plan will be automatically terminated (For the Formal sector).
We will de-activate/suspend your account on failure to renew the health insurance.
You shall be assigned health care facility within your place of residence.
You agree to provide all required information upon registration which must be accurate and must be duly notified in event of any change.
At the point of registration, you will be issued with IMSHIA Identification Card.
You agree to safe keep the IMSHIA Identification Card and in the event of any loss, misplacement, theft, damages, a prescribed fee to be determined by us shall be paid for re-issuance.
The IMSHIA Identification Card is not transferable at any point.
You are to use the IMSHIA Identification Card only at the health care facility assigned to you.
You have a waiting period of one month from the date of registration before you can access care from your assigned health care facility.
The maximum in-patient days in your assigned health care facility for medicine and surgery is 15 days and 30 days respectively, after which the entire bill shall be funded by you.
We do not have any Agent, Referee, Organization for collection of any fee and cannot be held liable in the event of any crime; hence, all payment must be made into IMSHIA account and there is no refund of money after payment.
You are expected to make payment of 10% of the total cost of drug charges.
You are to make co-payment in some services offered to you.
